CLIENT GRIEVANCE NOTIFICATION
You have the right to complain if you are not happy. You have the right to grieve if you believe your rights have been violated.
HOPE in Fostoria has an internal client complaint system in which, in our experience, most complaints are resolved quickly and satisfactorily. We request you address your complaint with your direct service provider first. If it is not resolved to your satisfaction, ask for a manager.
If you believe your rights have been violated ask to speak with a Client Advocate or complete a Grievance form which is available upon request from any agency staff member.
In either case, your complaint or grievance will be promptly investigated, and action will be taken to resolve it. Should resolution not be attained at this level, your case will be referred to the Board President of HOPE in Fostoria.
